PAX8 (paired-box protein 8) belongs to the family of transcription factors. It is structurally similar to PAX5 and PAX2. PAX8 is expressed in an orderly manner during fetal development. It is essential for organogenesis during embryonic development of the kidney, Müllerian duct, and thyroid. These transcription factors often disappear in mature tissues but can be re-expressed in an organ-specific manner during neoplastic transformation. Due to its restrictive expression in normal tissues, PAX8 is a sensitive and specific marker for primary tumors as well as for metastatic tumors originating from the aforementioned organs and tissues. Thus, PAX8 is a nuclear marker for epithelial neoplasms originating in the thyroid, thymus, ovary, endometrium, endocervix, fallopian tubes, or kidneys.
