NUT carcinoma (NC, formerly NUT midline carcinoma) is a rare, aggressive subtype of squamous cell carcinoma defined by a chromosomal rearrangement of the NUT gene (also known as NUTM1, nuclear protein in testis). It typically arises in the midline of the body, including the thorax, mediastinum, lungs (thoracic regions ~50%), and head and neck (~40%), but has also been diagnosed outside the midline, including the salivary gland, pancreas, bladder, kidney, adrenal gland, and various soft tissue and bone sites. NC is a nearly uniformly fatal cancer with a reproducible median overall survival of 6.5 months. Although NC can occur at any age, it primarily affects adolescents and young adults, with a median age of 24 years. In the majority of cases (~75%), NUT is fused to BRD4. This creates a chimeric, potent BRD4-NUT oncoprotein. Various NUT fusion partners, including BRD3, NSD3, ZNF532, and ZNF592, encode proteins that interact with BRD4 and serve to connect NUT to BRD4. The diagnosis of NC can be made by positive immunohistochemical staining of the NUT nucleus.
