Ep-CAM, also known as Epithelial Specific Antigen, is a transmembrane glycoprotein involved in cell adhesion in epithelia, cell signaling, proliferation, differentiation, and migration. The glycoproteins are located on the cell membrane surface (preferably basolaterally) and in the cytoplasm of virtually all epithelial cells, with the exception of most squamous epithelia, hepatocytes, proximal tubule cells of the kidney, parietal cells of the stomach, and myoepithelial cells. However, focal positivity can be detected in the basal layer of the squamous epithelium of the endoderm (e.g., palatine tonsils) and mesoderm (e.g., cervix). In liver lesions such as hepatitis and cirrhosis, hepatocytes are often Ep-CAM positive. Normal mesothelial cells are Ep-CAM negative but may show a focal reaction in reactive changes. Mesenchymal cells and neural crest cells are negative, with the exception of olfactory neurons. Ep-CAM is overexpressed in certain carcinomas. Ep-CAM is found in the vast majority of adenocarcinomas of most locations (50–100% in various studies), as well as in neuroendocrine tumors, including small cell carcinomas. Renal cell carcinomas and hepatocellular carcinomas are stained in approximately 30% of cases. Therefore, this antibody can be a diagnostic marker for various cancers, including adenocarcinomas, neuroendocrine tumors, renal cell carcinomas, hepatocellular carcinomas, and carcinomas of the colon, pancreas, and breast. It is a sensitive marker for basal cell carcinomas.
