Keratins are cytoplasmic intermediate filament proteins expressed by epithelial cells. Cytokeratins 5 and 6 are type II keratins that, in combination, are useful for distinguishing mesotheliomas (positive) from lung carcinomas (negative) or metastatic carcinomas in the pleura (negative). The cocktail detects undifferentiated large cell carcinomas, squamous cell carcinomas, spindle cell squamous cell carcinomas of the skin, malignancies of the prostate and breast, and helps differentiate between ductal hyperplasia of the breast (strong staining) and solid papillary DCIS (negative).
