Keratins are cytoplasmic intermediate filament proteins expressed by epithelial cells. Cytokeratin 5 is a type II keratin, and cytokeratin 14 is a type I keratin. CK5 is detected in metastatic carcinomas in the pleura, epithelial mesotheliomas, almost all squamous cell carcinomas, half of transitional carcinomas, and many undifferentiated large cell carcinomas. CK14 is found in basal cells of squamous epithelia, some glandular epithelia, myoepithelia, and mesothelial cells in various tissues, including the prostate and breast. A cocktail of CK14, its cognate partner CK5, and p63 is useful for detecting the basal-like phenotype of breast carcinoma and for distinguishing normal prostate from prostate cancer.
